Canterbury and Westland Inc. Total Mobility Taxi Vouchers. To obtain the total mobility taxi vouchers: first there is an assessment, which is carried out either at hospital or at the amputee's home. Once an ID card has been issued, two voucher books are also issued. When the first book is empty, it should be posted back to the Assessor with a stamped self-addressed envelope and a new book is then posted out. For enquires or and assessment contact:.
