Blood conservation in the surgical patient is a team effort that requires careful planning throughout the patient’s entire hospital stay. Many blood conservation devices and techniques have been developed to decrease and/or eliminate the need for blood transfusion in the surgical patient. Laboratory screening and identification of high-risk patients. Pre-Operative identification of anemia. Treatment of anemia may include: Iron, Folate, B12, EPO. Ensure availability of equipment and personnel.

1 g/dl increase Hb. 3% increase in Hct. Need to increase O2 carrying capacity. 3x10 pltsPool random donors (RDP). V single donor pheresis SD. Assay 10 min- 1 hr post transfusion Increase in count. Plt 10 20,000/mL. Plt 50,000/mL with bleeding or invasive procedure. Plt 100,000 CNS surgery. Platelet dysfunction with bleeding. Decrease in PT, INR. PT 15 x the upper limit of normal or the midpoint of normal range; aPTT 1.5 x the upper limit of normal with bleeding or invasive procedure. 1:1,600,000 HIV.
