About Jay Jalil Nassirzadeh - Owner/Administrator. My family and I have owned and operated the Phillips Manor Nursing Home since 1999. During this time, Phillips Manor was rated as the best nursing facility in the Greater Lynn area twice. I am licensed as a nursing home administrator in both Massachusetts and Rhode Island. PT/OT/Speech Therapy

Physical Therapy, Occupational Therapy and Speech-Language Pathology services are offered. These services are available 7 days per week, 365 days per year. The Rehabilitation Department is involved in the skilled care of residents with a variety of needs and diagnoses (short-term rehab/sub-acute care/long term care). All residents are screened and evaluated by a therapist within 24 hours of referral. Rehabilitation professionals are involved in care planning, family meetings and discharge planning.

Payment for services is achieved through the use of Medicare, Medicaid, and Private pay. One may qualify for up to 100 days of skilled nursing and/or rehabilitative care if she/he is:. 65 or over or disabled. Has Medicare Part A benefits. Has been hospitalized for at least 3 midnights prior to admission. Also known as MassHealth in Massachusetts, it is available for those who have exhausted their own personal resources and need state assistance to help pay for their care. We also accept Private Pay.

Facilitate recovery through short term rehabilitation and to aid in the transition from hospital to home.
